I , grant MoTail 6 permission to use my dog or myself likeness for future advertising or entertainment purposes via print, video, television, internet, radio, and social media including but not limited to; Facebook, Instagram, Twitter, You Tube.

    I recognize that all activities at MoTail 6 are recorded for said purpose.

    Should MoTail 6 receive any monetary gain for myself or my dogs likeness appearing on said platforms and hosted by MoTail 6 or its representatives, I agree That I am entitled to no compensation. If any payments are to be made the amount is to be determined at the sole discretion of MoTail 6
